PASRR is an Important Requirement
Individuals with a serious mental illness, intellectual disability, or a related condition have special protections under state and federal law to ensure that long-term services and supports are provided in the right setting to meet their needs. Acentra Health, formerly Kepro, contracts with the Agency for Health Care Administration (AHCA) to provide Preadmission Screening and Resident Review (PASRR) Level I and Level II services for the state of Florida.
Acentra's PASRR program provides protections that align with federal and state laws under the Americans with Disabilities Act (ADA) to ensure that anyone with a serious mental illness, intellectual disability or a related condition is served in the least restrictive setting. We partner with hospitals and nursing homes throughout the state of Florida to provide Level I screenings as well as Resident Reviews when there has been a significant change in a residentās condition.
Acentra provides an online portal to nursing facilities and hospitals that allows for simple and secure submissions; trainings to assist users in their roles; and technical assistance with one-on-one sessions.

In order to use the Provider Portal, each nursing home and hospital should appoint one person to be the Administrator, or owner, of the provider portal account. The account Administrator is typically a supervisor as this user role holds the highest system permissions.
There should be a minimum of one Administrator per facility.Ā The Administrator is able to create additional users. There are three types of users that can be created by Administrators: additional administrators, Level I screeners, and Level II requesters.

PASRR Level I Screenings
Acentra Health, formerly Kepro, contracts with AHCA to serve as the Florida PASRR Level I and Level II vendor. For individuals residing in a community setting, Acentra has been delegated by AHCA to complete an onsite PASRR Level I screening.
Level I screeners must be an MSW, LCSW, LMHC, RN, ARNP, a psychologist, or a physician. If the PASRR Level I outcome is positive, then a PASRR Level II evaluation and determination must be completed prior to a Medicaid-certified nursing home admission, regardless of payor source (CFR 42-Ā§483.102 and 59G-1.040, F.A.C).
PASRR Level II Requests
Acentra Health, formerly Kepro, has been completing PASRR Level II evaluations and determinations for the state of Florida for individuals with serious mental illness since 2007, and for individuals with intellectual disability and related conditions since 2016.
Acentra Health utilizes a multidisciplinary approach. Acentra's professional staff, which includes psychologists, nurses, social workers, and physicians, work together to evaluate the appropriateness of nursing facility placement and the need for specialized services for each individual referred for PASRR Level II evaluation and determination.
Acentra Health adheres to the federal and state regulations governing the PASRR process. The Level II evaluations are sensitive to each individualās culture, language, ethnic origin, and means of communication.
When nursing home level of care is found to be needed, the program will work to ensure that the patientās mental health needs, and any other disability needs, are met. This means providing recommendations on rehabilitative services or specialized services, and tracking the progress of the individual to ensure that their changing needs are continuously met.
Required Documents & Forms
According to 59G-1.040, F.A.C., the following documents are required for a PASRR Level II evaluation and determination.
- Completed PASRR Level I Screen or Resident Review for Serious Mental Illness (SMI) and/or Intellectual Disability or Related Conditions (ID) (Completed online via the provider portal.)
- Informed consent, as documented on the Level I PASRR Screen (Completed online via the provider portal, printed, and signed.)
- AHCA-5000-3008
- Other current and relevant medical documentation Including:
- History and physical
- Relevant case notes or records of treatment
- Medication administration records
- Psychiatric or psychological evaluation, if available
- Minimum Data Set (MDS), if available
Reconsideration Requests
Nursing facilities, hospitals, and other providers are able to request a reconsideration review of any adverse determinations (as defined in 42 CFR Part 431.201) received for individuals under their care. Adverse determination means a determination made in accordance with sections 1919(b)(3)(F) or 1919(e)(7)(B) of the Act that the individual does not require the level of services provided by a nursing facility, or that the individual does or does not require specialized services.
If an individual disagrees with a determination, that person, or a provider on their behalf, can request a reconsideration of the determination by contacting Acentra Health, formerly Kepro, within 30 days of receipt of the determination notice.
