Florida PASRR Level I Screening Information
Nursing homes and hospitals do Level I screenings. If the Level I is positive, Level II evaluations and determinations are done. These are done before a person goes to a nursing home.

Florida PASRR Level II Information
Acentra Health, formerly Kepro, works with the AHCA to do Level II evaluations and determinations. These are done for any Medicaid-certified nursing facility applicant and resident. The Level I screener determines the diagnosis.
Level II evaluations and determinations can include a plan of care. A plan of care is made when specialized services are needed. These services provide additional help for people in nursing homes.

Community & Home Care Options
People have choices about where they live and where they receive care. PASRR can help people find support in the community. People can get assistance with maintaining or achieving the highest level of independence possible.
Placement choices for people with long-term care needs include services of care for community settings. These may be offered to nursing home residents.

- Community mental health – partial hospitalization program
- Comprehensive outpatient rehab facility

Reconsideration Requests and Appeals
Have you recently received a screening from us? We screen for mental health and learning issues. This helps us find out if you need special care. This care might include a nursing home. If you disagree with our decision, you have rights. You can request a new review. You must request it within 30 days. This period starts when you received our letter. To request a new review, contact us using the information below.

If You Request a New Review
We will send you a notice about our results. You will receive this within six days. This does not include weekends.
If You Disagree with This New Decision
You have rights and you can request a hearing. You must request it within 30 days. This period starts when you receive our second letter.
